The foundation of sound industrial safety and operation is sound human performance — human performance free of consequential human errors.

PII’s 25-year research and analysis of more than 80,000 injury and operational event data has yielded a detailed understanding of critical controlling parameters that affect all aspects of human errors. With this understanding and newly developed Error-Free® technology, PII is able to help companies, organizations, or programs to achieve Error-Free® operations in a short period of time. PII assures that this Error-Free® technology could practically eliminate consequential errors by a minimum factor of ten reduction in consequential error rates. Using this groundbreaking technology, PII’s clients enter into and stay in The Error-Free Zone® with practically zero injuries or events. In the Error-Free Zone®, workers are free from the fear of unexpected injury, and companies could avoid costly consequences of unexpected operating events.

With this Error-Free® technology, PII can quantitatively assess an organization, a program, or a company, and determine the weakest links of the system and their contribution to poor performance (manifested as injuries and/or operational events). Just doing quantitative assessment, however, is not enough to ensure Error-Free® operation. PII follows the assessment with cost-effective fixes for inadequate LOPs (layers-of-protection), and with tailored Error-Free® training for the workforce. The training courses, packed with real-life data and examples, are tailored to the organization based on its specific levels of work, experience, knowledge, age, and working environment. Our solutions are not one-size-fits-all, and are work-group specific.

Some critical controlling parameters that affect human errors were extensively studied quantitatively in the Error-Free® technology. The results are factored into PII’s quantitative assessment and Error-Free® training courses:

  1. Human Error Cycles
  2. Distractions, Drowsiness, Over-confidence, Time-pressure, Attention Bank and Span (DDOTA)
  3. Complacency and Group Think
  4. Trust and Betrayal
  5. Mindset
  6. Attitudes and Leadership
  7. Personality and Gender Effects
  8. Brain Abnormality (Dyslexia, Color-blindness, etc.)
  9. Motivation and De-motivation
  10. Communication Error Traps
  11. Age Effects
  12. Experience Effects
  13. Memory and Learning
  14. Environmental Effects
  15. Holiday Effects
  16. Behavior Modification and Rules
  17. Psychologically-Based Analytical Thinking
  18. Team Effects
